 Due to a setting in my server I have to login using: private key,
 password, google-authenticator. This used to work. After reinstalling my
 Mac, it doesn't anymore. After logging in with private key, I get the
 "Partial authentication success" where I can enter a Password (but not
 username, this field is blank). After entering the correct password, I get
 the error "Interoperability failure" "Change of username or service not
 allowed: (<<my-user-name>>,ssh-connection) -> (,ssh-connection). Please
 contact your web hosting service provider for assistance."

 The problem seems to be that the username isn't carried on to the second
 screen (where I enter my password). I can also not add/change the username
 here.
